Does my child qualify for hawk-i dental coverage?

To qualify, a child must live in Iowa and:

·        Be under 19 years old

·        Be a citizen of the United States or a qualified alien

·        Be in a family that meets the hawk-i income limits

·        Cannot be the dependent of a State of Iowa employee

·        Cannot qualify for Medicaid

Use these charts to see if your child can get free or low-cost dental care coverage.

 

If your family's yearly countable income is in the blue chart and your child does not qualify for Medicaid, dental coverage may be FREE for your children.

What services are covered?

The hawk-i dental program covers these and other dental services:

·        Cleanings and check-ups

·        Fluoride treatments

·        Sealants

·        Fillings and crowns

·        Medically necessary orthodontia.  (Prior approval is required)
