Does my child qualify for hawk-i dental coverage?

To qualify, a child must live in Iowa and:

        Be under 19 years old

        Be a citizen of the United States or a qualified alien

        Be in a family that meets the hawk-i income limits

        Cannot be the dependent of a State of Iowa employee

        Cannot qualify for Medicaid

Use these charts to see if your child can get free or low-cost dental care coverage.

 

If your family's yearly countable income is in the blue chart and your child does not qualify for Medicaid, dental coverage may be FREE for your children.


These rates go into effect for any application received on or after March 1, 2013 with an effective start date of April 1, 2013.
If your family's yearly countable income is in the blue, and you do not qualify for Medicaid, your children may be able to get FREE dental-only coverage under hawk-i.

What services are covered?

The hawk-i dental program covers these and other dental services:

        Cleanings and check-ups

        Fluoride treatments

        Sealants

        Fillings and crowns

        Medically necessary orthodontia. (Prior approval is required)
